Navigating Obstetric Challenges With Cord and Placental Abnormalities

This presentation will guide medical professionals through the diagnosis and management of third-trimester bleeding, with a focus on identifying obstetric causes such as placenta previa and vasa previa through ultrasound imaging. It will emphasize the critical role of prenatal screening in detecting conditions like vasa previa, which can drastically improve patient outcomes from high fatality risks to near-certain survival when appropriately managed with pre-labor cesarean delivery.

Learning Objectives:

  • Describe etiologies for late preterm bleeding associated with the placenta. Recognize the protean presentations of PAS. Define vasa previa and why it carries such a high risk of death to the baby.
  • Understand imaging features of the most common placental abnormalities associated with late term bleeding. Triage cases according to suspected severity/risk level. Recognize which patients are at risk for vasa previa.
  • Discuss pitfalls when imaging the placenta. Appreciate the importance of operative management in specialized PAS centers of excellence. Discuss the methods for screening and diagnosing vasa previa and review the steps to avoid perinatal death from vasa previa.
