Masses involving the fetal facial architecture are among the rarest of fetal malformations. The two most frequent masses that may obstruct the airway are lymphatic malformations and teratomas. Prenatal imaging is essential to predict the extent of the involvement of the airway, consider the potential need for an EXIT procedure, and to optimize plans for postnatal care. We present the case of a fetal facial mass noted on a third trimester ultrasound. Ultrasound, MRI, and multidisciplinary care were utilized to make a plan that resulted in a successful outcome.
