Evaluation Of Acute Monocular Vision Loss With Point-of-Care Ultrasound: A Case Of Central Retinal Artery Occlusion

Central Retinal Artery Occlusion (CRAO) is an uncommon but serious vision threatening and potentially life-threatening diagnosis often due to thromboembolic events. We describe a patient who presented to the emergency department with painless vision loss. Bedside ocular point-of-care ultrasound was performed and allowed for rapid diagnosis of CRAO with visualization of the ‘spot sign’ despite negative cerebral vascular accident work-up. This highlights the importance of high-quality bedside ultrasound skills to make the crucial diagnosis. By increasing knowledge and availability of POCUS in evaluation of acute ocular pathology, providers can increase recognition and potential intervention of vision threatening pathology including CRAO.
