The introduction of this concept in Guatemala represents a real challenge for obtaining knowledge and positioning of the resident doctor in comparison with other specialties. However, just 4 years after the introduction of this concept, it has been very useful for students in the area of shock where the pathologies are time dependent such as states of obstructive shock not associated with trauma, where differential diagnoses are usually hide the reality and ultrasound allows its early detection. Therefore, the case of a state of obstructive shock secondary to complications due to chronic kidney disease is presented.
