We describe a case of intermittent reverse end-diastolic flow in the MCA in a fetus with congenital heart disease. We also reviewed 51 published patient cases of reversed end-diastolic flow in the MCA and differentiated the clinical findings and subsequent perinatal outcomes based on intermittent versus persistent flow. Our study highlights this atypical finding and demonstrates the need for close antenatal follow-up when EDF reversal is noted in the fetal MCA.
