In this case series, we present antenatal imaging findings for two fetuses later diagnosed with autosomal recessive polycystic kidney disease (ARPKD). Both fetuses possessed classic sonographic features of ARPKD; however, they additionally were noted to have nephrocalcinosis, a rare finding in individuals with ARPKD. The combination of enlarged kidneys with increased echogenicity in combination with nephrocalcinosis should increase clinical suspicion for ARPKD antenatally.
