Most pregnancies complicated by the presence of a fetal abdominal mass go on to uncomplicated vaginal delivery. Abdominal dystocia has been described, most often reported in case of polycyctic kidney disease. We report the case of a large fetal abdominal mass which caused severe shoulder and abdominal dystocia, resulting in profound neonatal hypoxemia, This is the first reported case of dystocia cause by a urogenital sinus. abnormality When the fetal abdominal mass is very large and the abdominal circumference is > 350 mm, obstructive complications may be more frequent and for this reason, a cesarean should be considered.
